A patient with pressure-like chest discomfort, diaphoresis, regional ST depression, and an above-reference troponin is not a clinic chest-pain workup to complete at leisure. The safe frame is suspected high-risk non–ST-elevation acute coronary syndrome (NSTE-ACS) until proven otherwise, while actively screening for aortic dissection, pulmonary embolism, pneumothorax, and other time-sensitive mimics.
Consider a 57-year-old man with type 2 diabetes, hypertension, and active tobacco use who develops substernal pressure while walking. The discomfort persists at rest and radiates toward the jaw. He is anxious and sweaty; his oxygen saturation is 96%, but the ECG shows new horizontal ST depression in the lateral precordial leads. A point-of-care troponin is twice the assay’s upper reference limit.
Before labeling the event, quickly establish onset and progression, exertional capacity at baseline, prior similar symptoms, known coronary disease, heart failure symptoms, syncope, medication use, aspirin allergy, bleeding history, anticoagulant use, and recent phosphodiesterase-5 inhibitor exposure. If baseline function or prior testing is unknown, document that uncertainty rather than filling in the missing history.
Name the syndrome before naming the score
The working diagnosis is suspected NSTE-ACS, with type 1 myocardial infarction (MI) among the leading possibilities if plaque disruption is ultimately confirmed. ST depression and T-wave inversion can represent myocardial ischemia, but they do not by themselves prove infarction. Likewise, one elevated troponin establishes myocardial injury, not necessarily acute myocardial infarction.
An NSTEMI diagnosis requires an acute rise and/or fall in cardiac troponin, with at least one value above the assay-specific 99th percentile, plus clinical, ECG, imaging, or angiographic evidence of ischemia. Thus, the first elevated point-of-care result is highly concerning, but serial testing using the receiving hospital’s assay-specific protocol remains important.
HEART score: useful context, not a discharge certificate

The exact troponin point depends on the assay and local HEART implementation. More importantly, the score should not be used as a gatekeeper when the patient already has ischemic ECG changes, ongoing symptoms, and an elevated biomarker. In a family medicine clinic, an 8 supports what the clinical picture has already made clear: activate emergency medical services, transfer to the emergency department, and do not discharge for outpatient stress testing.
In the hospital, serial ECGs and troponins, cardiology assessment, and risk-based invasive management determine the next step. Intermediate- and high-risk NSTE-ACS generally warrants an invasive approach during hospitalization when the patient is an appropriate candidate for revascularization, but “high HEART score” is not synonymous with “automatic catheterization from the clinic.”
What to do while EMS is arriving
The order of operations matters more than memorizing a mnemonic.
- Call EMS and place the patient on continuous observation. Keep him at rest, obtain vascular access if trained staff and equipment are available, and ensure that the defibrillator and emergency medications are accessible. The patient should not drive himself.
- Obtain and transmit the ECG immediately. A repeat tracing may be useful if symptoms evolve, but additional clinic testing must not delay transport.
- Give non-enteric-coated chewable aspirin 162–325 mg if there is no active bleeding, true aspirin hypersensitivity, or another clear contraindication. Record the exact dose and time.
- Consider sublingual nitroglycerin for ongoing ischemic discomfort if he is hemodynamically stable, with systolic blood pressure at least 90 mmHg, there is no clinical concern for right-ventricular infarction, and he has not recently taken avanafil within 12 hours, sildenafil or vardenafil within 24 hours, or tadalafil within 48 hours. A typical regimen is 0.3–0.4 mg every 5 minutes for up to three doses, following local protocol. Nitroglycerin is not a diagnostic test.
- Do not give routine oxygen to a patient with an oxygen saturation of 96% and no respiratory distress. Give oxygen for confirmed hypoxia, generally an oxygen saturation below 90%, or when clinically significant respiratory compromise requires it; do not administer it routinely when saturation is at least 90%.
- Do not let analgesia or additional medications delay transfer. Morphine is generally reserved for pain resistant to other tolerated anti-ischemic measures and may delay the effect of oral P2Y12 therapy. Heparin and P2Y12 inhibitor use, as well as beta-blocker decisions, should be coordinated with the receiving emergency and cardiology teams rather than applied reflexively in clinic.
Keep the dangerous alternatives visible
The diabetic smoker’s risk profile makes ACS likely, but it does not make every chest pain syndrome coronary. Ask whether the pain pattern or examination changes the pathway.

Aortic dissection deserves particular attention because coronary involvement can produce ischemic ECG changes and troponin elevation. If the pain is abrupt and tearing, the examination shows pulse or pressure asymmetry, or there is a new neurologic deficit or aortic regurgitation murmur, communicate that concern before the receiving team commits to an antithrombotic pathway.
Common traps on exams and in practice
Calling NSTEMI from a single troponin. The correct language at the clinic door is “suspected high-risk NSTE-ACS with an elevated troponin,” pending serial values and the full ischemic assessment.
Treating the HEART score as a diagnosis. HEART helps stratify undifferentiated emergency-department chest pain. It does not make a high-risk patient safe for outpatient management, and it should not delay EMS activation.
Giving oxygen because the patient looks ill. Diaphoresis and anxiety are concerning, but a normal saturation without respiratory distress is not an indication for routine oxygen.
Using morphine as a reflex. Pain control matters, yet opioids may delay the effect of oral P2Y12 therapy. Transfer and definitive evaluation come first.
Over-trusting reproducible or “indigestion-like” pain. A benign-sounding descriptor cannot neutralize exertional symptoms, ischemic ECG changes, or troponin elevation.
After the acute event
Once the diagnosis and anatomy are clarified, secondary prevention becomes part of family medicine’s work. High-intensity statin therapy is recommended for ACS, while antiplatelet selection and duration depend on PCI, CABG planning, bleeding risk, and the need for anticoagulation. Smoking cessation, blood-pressure control, diabetes management, medication reconciliation, cardiac rehabilitation, and timely follow-up are not add-ons; they are part of preventing the next event.
