An elevated office blood pressure is not always a cue to add another drug. In an older adult who is dizzy, uses pill color rather than labels, and arrives without transfer records, the immediate threat may be medication-related harm disguised as resistant hypertension.
In this illustrative case, consider a 74-year-old woman who recently moved and brings a shopping bag of prescription bottles, over-the-counter ibuprofen, and St. John’s wort. The first question is not “What should I add?”
Start by asking the patient whether she wants her daughter involved in the entire discussion, only medication organization, or selected decisions. Reconcile before you optimize
Use the bottles, pharmacy dispensing history, prior records, and the patient’s actual routine. Record the indication, dose, timing, last dose, missed doses, and reason for every medication, including supplements and products labeled “natural.” A pharmacist-led review is especially valuable when the patient has CKD, polypharmacy, and difficulty identifying pills.
The most immediate medication concerns are not identical in urgency:
- Oral ibuprofen: Recommend stopping self-directed oral NSAID use while a safer osteoarthritis plan is arranged. NSAIDs can reduce renal blood flow, worsen blood pressure control, and increase the risk of acute kidney injury or CKD worsening; the concern is especially important with CKD and renin-angiotensin system blockade. If she has type 2 diabetes and confirmed CKD, consider an SGLT2 inhibitor promptly once medication use, hydration, and orthostatic symptoms are stable; current guidance supports initiation at an eGFR of at least 20 mL/min/1.73 m² when clinically appropriate. This is a staged optimization decision, not a reason to introduce another pill before the first-visit safety problems are addressed.
The goal is not simply the fewest medications. It is the simplest regimen that preserves meaningful benefit and minimizes avoidable harm.
Targets should follow function, not age alone
An A1c of 8.2% may be close to an appropriate goal for an older adult with multiple chronic illnesses and limited self-management capacity, but that conclusion requires a functional and cognitive assessment. The patient’s pill confusion is a signal to investigate, not proof of dementia. Review hypoglycemia history, glucose patterns, nutrition, falls, life expectancy, and treatment preferences before setting a target.
Similarly, a seated systolic pressure in the 150s deserves attention, but symptomatic orthostasis changes the immediate priority. The practical goal is a tolerable, sustainable blood-pressure plan that protects the kidney and cardiovascular system without provoking falls or syncope. Do not use a single office value to justify escalation when measurement technique and adherence remain uncertain.
Follow-up is part of the prescription
This patient needs prompt follow-up rather than a routine three-month return. Recheck creatinine, eGFR, and potassium after removing NSAID exposure and clarifying the regimen; obtain urine albumin testing and review a simple home glucose and blood-pressure record. Confirm that the home cuff is appropriate, that readings are taken after rest, and that standing symptoms are recorded alongside the numbers.
Simplify the regimen with one pharmacy, synchronized refills, large-print instructions, a medication chart using names and purposes rather than color alone, and a blister pack or pill organizer if the patient can use it safely. Use chunk-and-check teach-back: “We covered several changes. Please show me which pills you will take tonight and tell me what you will do if you feel faint.” If the answer is incorrect, re-explain in a different way and ask again.

- Making several medication changes without a clear follow-up laboratory plan and teach-back.
