ABFM Family Medicine Certification Examination Guide
The ABFM Family Medicine Certification Examination has 300 single-best-answer questions in four 75-question sections, with 95 minutes per section and 100 pooled break minutes. Use the five-domain blueprint to guide preparation, and confirm initial-certification requirements for your specific pathway.
The ABFM exam for initial certification
The American Board of Family Medicine (ABFM) uses the Family Medicine Certification Examination, also called the one-day exam, as an examination requirement for initial certification. The same examination is also given to continuing-certification candidates; this guide focuses on initial certification.
The exam assesses family medicine knowledge and clinical problem-solving across a broad scope, including adult, maternity, gynecologic, pediatric and adolescent, behavioral and mental health, surgical, older-patient, and community medicine. Diagnosis, management, and prevention are represented.
Passing the exam alone does not complete initial certification. ABFM also verifies the training requirements for the candidate's pathway, required licensure, and continuing compliance with its professionalism requirements. Other requirements, including certification activities, depend on the pathway.
Format, timing, and section navigation
The exam is computer-based and administered through Prometric. All questions use a single-best-answer multiple-choice format.

The standard schedule provides three optional breaks: two 15-minute breaks and one 70-minute meal break. Candidates can distribute the pooled break time differently; once it is used up, further break time reduces the time remaining for the exam.
During an active section, candidates can navigate among questions, mark items, and review or change answers. A section cannot be reopened after it ends or times out, so complete any review before the section closes.
Five clinical-activity domains
ABFM's current blueprint is organized around clinical activities rather than organ-system categories. It applies to the one-day examination beginning in 2025. Use the target percentages to plan relative study emphasis while maintaining broad family medicine review.

These percentages are blueprint targets, not a reason to omit smaller domains or other family medicine topics.
Initial-certification requirements depend on pathway
ABFM has distinct initial-certification routes. The points below describe common pathway differences, not a universal eligibility rule. Verify your own requirements in the current Examination Information Booklet and MyABFM application.

For residents, exam-window eligibility can depend on expected training-completion timing. A passing result is only one part of certification; ABFM must also verify the other applicable pathway requirements.
A practical preparation plan

Prioritize by blueprint, not only by organ system. Start with Acute Care and Diagnosis, Chronic Care Management, and Emergent and Urgent Care, while reserving dedicated time for Preventive Care and Foundations of Care.

Practice the decisions reflected in the domains. Work on single-best-answer questions involving diagnostic next steps, diagnosis, initial treatment, chronic management, urgent decisions, prevention, and care-delivery foundations.

Rehearse the section pace. Practice blocks of 75 questions in 95 minutes. That averages about 76 seconds per question; flag difficult items and return to them while the section remains active.

Keep an error log by domain and decision type. Note whether a miss came from recognizing the problem, choosing the next step, selecting management, or applying a prevention or foundation principle. Use recurring patterns to guide review.

Practice the interface and review rules. Use ABFM's tutorial to rehearse navigation and marking. Review or change answers before each section ends; completed sections cannot be reopened.

Plan the pooled breaks. Decide how you will use the available break time, including the meal break, and avoid letting one item or break consume time needed for the remaining sections.
